Why exam not covered to diagnose problem

I was surprised to see that the exam to determine that Nate had a double ear infection was not covered. How can you diagnose a problem with doing an exam? It should be covered.

Claim payment

I do not feel that you are paying correctly. SM has a seizure symptom and you are not paying for the Keppra until it reaches a deductible by itself which is not correct since it is part of the SM condition.
