Need instrucitons on how to obtain diagnosis info

Fairly easy to submit claims but I've received multiple claim responses asking for diagnosis information. When I've provided it, I still don't get the coverage. Please explain what Nationwide needs in order to satisfy the requirement and get the reimbursement. This claim has not been filed yet, trying to find out from vet what I might need. Thanks, JoLyn

I appreciate knowing Nationwide was there

When your furry kid is sick and not wagging his tail or eating, it’s very stressful. It’s nice to know there is a bit of financial relief when the medical bills increase as treatments come along. He is doing much better now. Whew!

Nationwide has helped me to keep Josh healthy ....he is 13 and quite spry

Nationwide is a great company....I have had many issues with Josh and if it weren't for Nationwide I would still not be fortunate enough to have him. Every pet owner should really try to get insurance as it is the only way to insure good care for your animal. Vets are very, very expensive

Coverage

Nationwide offers a variety of plans offering varying levels of coverage. The company’s most popular plan, Whole Pet with Wellness, offers 90% reimbursement for accident, injury, illness and wellness care, inclusive of exam fees. All Nationwide members have complimentary 24/7 access to vethelpline.

Payout Limits

Depending on the plan selected, there may be annual benefit limits or per incident benefit allowances. There is no annual limit for the company’s most popular plan, Whole Pet with Wellness.

Deductible

Annual deductibles range from $0 to $1,000. Most popular annual deductible for Whole Pet with Wellness is $250.

Age Limits

No age restriction to enroll in company’s most popular plan, Whole Pet with Wellness. Other plans may have age restrictions.

Waiting Period

14-day waiting period after application approved and payment of premium is received.

Exclusions

Pre-existing conditions are not covered on any plans (if a condition is curable, it may be eligible for review if pet remains treatment-free for at least six months). Whole Pet with Wellness covers nearly all veterinary expenses. Notable exclusions include boarding and grooming.
